We are one of NHS England’s pioneer sites, with ambitious plans to develop a more integrated approach to care within west Norfolk, enabling us to improve the experience of patient care and ensuring the best possible health and wellbeing outcomes for our local community. To do this, we work with a range of partner agencies and individuals, including our local NHS Acute and Provider Trusts, family doctors, the borough, district and county councils, the voluntary sector, Commissioning Support Unit and, of course, our patients.
